For families
You can refer yourself
You do not need a referral from a doctor to get in touch with us. If you are wondering whether we are the right fit, you are welcome to simply ask.
To get started, send us an enquiry or give us a call. We will ask a few questions about what is happening, talk through the options and let you know what we can offer and what the next step looks like.
For professionals
Referring a client
We welcome referrals from GPs, allied health and community service providers, government agencies, WorkCover and self-referrals.
Whether you use the form below or email us, it helps to include:
- ✓The person’s name, date of birth and contact details
- ✓Their consent for you to share their details with us
- ✓The reason for the referral and any goals
- ✓The funding pathway, whether Private, NDIS or WorkCover
- ✓Any relevant reports, plans or assessments
The process
What happens next
Referrals usually follow these three steps.
We receive your referral
We will be in touch to confirm we have it, and to ask for anything further we need.
We contact the person
We get in touch with the person or their family to talk about what they are after and answer any questions.
We agree on the next step
If we’re the right fit, we confirm the service, the funding pathway and the timeframe that suits.
Wait times vary depending on the service and the time of year. If a referral is urgent, please tell us on the form or call us so we can talk it through.
Referral form
Send a referral
This form is for referring another person. If you are getting in touch about yourself or your own family, the enquiry form is the easier place to start.
Would rather send it directly?
You are welcome to email a referral, or call us and we will take the details over the phone. If you are sending documents, please make sure you have the person’s consent first.
